Please accredit our mass media organization to the event below.
- Name and date of the event for which accreditation is requested
- Name of media organization
- Type of media (newspaper, magazine, radio station, TV company, web-based media, etc.)
- Name of the head of the media organization, telephone, e-mail
- Journalist's name, position, mobile number, e-mail
- Actual address of the editorial office
- Website, telephone and fax numbers of the editorial office
- Planned date (dates) of publication
- Equipment (name, identification number) - if necessary
- Main subject of the request
- Preliminary list of questions
- Aims and objectives of the photo and video recording
- Proposed location and objects to be filmed
- Where the footage will be used
